Sənədin sorğu edilməsi / Requesting documents

Azərbaycan Respublikasından Baş Konsulluq vasitəsi ilə sənədi sorğu etmək üçün tələb olunan sənədlər

1. Doldurulmuş Forma;
2. Baş Konsulluğun adına yazılmış Ərizə;
3. Dövlət rüsumu - 10 ABŞ dolları. Dövlət rüsumu “money order” (nümunəyə bax) və ya “cashier’s check” (nümunəyə bax) formasında qəbul edilir (Payable to the Consulate General of Azerbaijan). Sorğunun nəticəsindən asılı olmayaraq bu rüsum geri qaytarılmır;
4. Sorğu edilən sənədin və ya sorğu ilə bağlı müvafiq sənədlərin surətləri (əgər varsa);
5. Sənəd sahibinin nümayəndəsi tərəfindən sorğu edildikdə, həmin şəxsin doğum şəhadətnaməsinin və ya pasportunun surəti.

Diqqət! Sənədlərin poçt vasitəsi ilə geri göndərilməsi üçün müraciətə poçt xərcləri ödənilmiş zərfin əlavə edilməsi xahiş olunur. Zərfin üzərinə dəqiq ünvanınızın yazılması və ödəniş markasının yapışdırılmasına nəzarət edin.

Sənədlərin poçt vasitəsi ilə göndərilməsi zamanı itirilməsi və yaxud yararsız hala düşməsinə görə Baş Konsulluq məsuliyyət daşımır.

Sənədlər tam və düzgün tərtib edilmədiyi təqdirdə icraya qəbul olunmadan ərizəçiyə geri qaytarılacaqdır.

Requesting documents through the Consulate General of the Republic of Azerbaijan

1. Completed Form;
2. Letter to the Consulate General;
3. State fee - $ 10 "money order" (see sample) or "cashier's check" (see sample). Payable to the Consulate General of Azerbaijan. This fee, regardless of the result of the request, is not refundable;
4. Copies of the requested or related documents (if available);
5. Copy of the birth certificate or passport of the person, if requested by the representative.

Attention! A prepaid return envelope is required. Make sure you write your exact address on the prepaid return envelope.

The Consulate General is not responsible for the loss of or damage to the documents during shipping.

Documents will be returned to the applicant if they are not properly compiled.
